New Clients

 

Northern Animal Clinic - LinzNAC always welcomes the opportunity to see new patients and make new friends. We ask that you contact our office to schedule an appointment. One of our Client Service Specialists would be happy to help you.

When you contact our office, you will be greeted by one of our Client Service Specialists. They will ask you a few questions: your name, address, phone, pet's name and age, and the reason for your visit.

Once we have scheduled your appointment, we will mail you a New Client welcome packet. This will include a new client form and payment options letter. These will need to be completed and brought with you to your scheduled appointment. We also include our clinic brochure and other information pertaining to your visit so you can be well informed before you get here.

Chalupa_1.jpgWe ask that you arrive about 10 minutes early for your appointment. This gives our team time to gather all your personal information and make sure all the forms are completed and entered into our computer system. If your pet has been seen at another veterinary facility, please bring all previous medical records. These records are very helpful to our team, because they provide insight with respect to previous medical conditions and medications. If you do not have a copy of these records we would be more than happy to contact your previous veterinarian and have a copy faxed to us.
